Referrals

Patients are normally referred to this office by their dentist, dental specialist, primary care physician or medical specialist.  If your medical insurance requires a formal referral (as in most HMOs), you will need to get that referral from your primary care physician before your appointment.

Insurance

Dr. Mitchell participates with most medical insurance companies including Anthem, Cigna, Harvard, and now also including Aetna. Individual policies vary with regard to coverage for certain treatment modalities.  Once treatment is recommended by the doctor, our office will help you find out what your policy will cover.  Please bring your insurance card with you at your first visit.

Patient Information

When you first come to the Center, you will be asked to fill out a few forms.  If you would like to do this before your visit and at your convenience, some of these forms can be downloaded from this site with Adobe Reader (pdf file) and then printed.
